Electronic medical record (EMR)
An electronic medical record (EMR) is a digital record of a person's health information used by clinicians and staff in a care organization.
Why it matters in practice
People and vendors sometimes use EMR and EHR interchangeably. Some definitions distinguish an EMR as organization-specific and an EHR as designed to support information across care settings, but usage is not perfectly consistent.
Example
Illustrative example: a clinic's staff use its EMR to document an encounter and review information recorded by that organization.
