Electronic health record (EHR)

An electronic health record (EHR) is a digital record of a patient's health information created over time and used across care settings.

Why it matters in practice

Depending on the system, an EHR may include information such as visit notes, medications, allergies, test results, and treatment history. The data available and how it can be shared vary by organization and implementation.

Example

Illustrative example: an authorized clinician reviews a patient's current medication list and visit history in the practice's EHR before an appointment.
