Patient matching

Patient matching is the process of identifying and linking health information that belongs to the same person within or across records.

Why it matters in practice

Organizations may use multiple demographic fields to compare records. The process must account for incomplete, inconsistent, or changed information and for the risk of matching the wrong person.

Example

Illustrative example: staff compare several available identifiers on an incoming referral with the practice record before attaching the document.
