Referrals

Closed-loop referral

A closed-loop referral is a referral whose completion and outcome are communicated back to the referring care team.

Why it matters in practice

Sending a referral is only one step. The referring office also needs a way to know whether the patient reached the specialist and whether information from that visit came back. The exact process depends on the organizations and systems involved.

Example

Illustrative example: a primary care office sends a referral, the patient completes a specialty appointment, and the specialist's visit information is returned to the referring team.
